'As boys, we are told to be brave': men on masculinity and mental health

New analysis found links between so-called masculine traits and depression and substance use. We asked men about gender stereotypes and mental health

E fforts have been made to challenge traditional stereotypes, but there is still pressure on men to be strong, independent, stoical, competitive and tough. According to research published by the American Psychological Association, these “masculine traits” have been linked with mental health issues such as depression and substance abuse .

Joel Wong, who led the research team at Indiana University Bloomington, said: “In general, individuals who conformed strongly to masculine norms tended to have poorer mental health and less favourable attitudes toward seeking psychological help, although the results differed depending on specific types of masculine norms.”

The research collated results of more than 70 US-based studies involving more than 19,000 men over 11 years. It focused on the relationship between mental health and conformity to 11 masculine norms. They included a desire to win, need for emotional control, and risk-taking.

The traits most closely linked to mental health problems were playboy behaviour or sexual promiscuity, Wong said.

We asked a group to talk about their own experiences of mental health and what they thought of this new analysis.

Depression feels like there’s a fog surrounding me. It is closing in and I cannot see what’s in front of me. It’s all-consuming and nothing else matters. I can see the fog building but I am powerless to stop it.

I experienced depression for about a decade before accepting what was going on. As a man it’s definitely much harder to talk about your emotions. It was my wife who made me go to the doctor. I had been behaving erratically: talking about killing myself, disappearing off for hours and leaving her worried sick. I was eventually diagnosed with clinical depression.

The pressure to uphold the traditional idea of masculinity stopped me getting help earlier. I come from the north and used to work in a shipyard. Among those I worked with there was an old-fashioned view of being a man – we don’t talk about our feelings. If you get upset about anything other than football you are considered a “sissy”. When people say, “How are you doing?”, if you say “A bit crap” the classic response was: “Could be worse.”

It was the same with older members of my family, who’d say, “Just get on with it”. Don’t complain or talk about what’s going on with you. It took me a long time to open up. Even going to a counsellor was difficult; talking about feelings was something women did.

But talking was exactly what I needed to do, and I am now training to be a counsellor. I lost a lot of friends, especially male ones, opening up about my depression. There’s a huge stigma around it. Doing counselling and getting involved with local charities, people have said talking to me made them feel better. I’ve had men thank me because it made them realise they are not alone. Men need to accept that talking is good and that the hardest part is showing emotion. Walking around with a smile on your face, telling jokes – that’s the easy part. But talking about life and your feelings, rather than how your football team did or how many pints you drank on Saturday night, is much harder.

I have suffered from poor mental health since I was about 16; I remember this vividly because I began to self-harm. It was also around this time that people started saying to me, “You need to talk to someone”. It took three years, two failed relationships and one botched suicide attempt for me to finally take this advice.

Looking back now, aged 25, I can see why, like many men, I found accessing mental healthcare so difficult. Masculine ideals of self-reliance, stoicism, sexual promiscuity and dominance over women do not lend themselves to emotional vulnerability. But it’s important to realise that this is not a mindset that suddenly appears. Rather, it’s instilled into men from a young age. I have personally lost count of the times I have heard parents of young boys admonish their own or other people’s sons for being “too sensitive” or “soft”. So acknowledging and addressing how we raise boys is vital here.

We must not dismiss the role culture plays in how men view and approach mental health. When I was diagnosed with borderline personality disorder there was one clear message my African parents wanted me to understand: you keep this to yourself. For years I was discouraged from getting help and actually felt guilty when I did. For many black communities taboos around mental health undoubtedly exist. But more than this, there is the awareness within these communities that structural racism means black boys, as well as black girls, are already at a societal disadvantage. Higher unemployment rates, longer prison sentences, lack of representation in media, politics and business and numerous other statistics will attest to this. The fear that poor mental health will simply be another stigma for black men to have to fight, coupled with many preexisting taboos, makes for a very tough battle for many.

With all this in mind, I often ask myself what can be done to help. I do not believe any man wants to feel emotionally closed off, lonely and isolated and I know they don’t want to be taking their own lives at the rate they are. By looking at the most sacred commandment of alpha masculinity, I believe it is possible to start to allow many men to address what they truly value, ask whether they are living according to their own standards and hopefully in this way begin to form a healthier relationship with their mental health.

Over the years I’ve had bouts of depression and anxiety. I’ve experienced severe sleep problems and suicidal thoughts. With years of psychotherapy I now know that all these are symptoms of complex post-traumatic stress disorder (PTSD). This is like PTSD but instead of being caused by a single traumatic event with flashbacks, it is caused by chronic or long-term exposure to emotional trauma.

My mental health has been helped almost exclusively by strong friendships and professional relationships with women and their empathy towards me. Had I been sexist, I can’t see how these important relationships would have developed. So I imagine there is a correlation with sexism and poor mental health, but I doubt that “masculinity” is the cause of poor mental health, rather that classical masculinity, which involves objectifying others, is non-empathetic and perhaps indicates other issues. I would imagine there is a similar correlation with racists and poor mental health. So I would consider the key factor to be one that is not exclusive to men, but affects all people who severely lack empathy and isolate themselves as a result.

In the past I’ve maybe found it harder as a man to talk to people about mental health, but not nowadays. I used to find it harder, because to talk, you need a support network but with severe mental health problems you can scare people off and have no support. Now I have this and I have different people to talk to. But to have empathetic friends, it really helps to be empathetic too, so you can share problems.

It can be very hard to admit vulnerability. Perhaps being a fairly big guy it’s easy to appear manly, to play that role, but the traits of “manliness” leave so little space for other important elements of our humanity. Traits that society might portray as more “feminine,” such as compassion, understanding and care, are essential, and sources of great strength.

Men feel they aren’t allowed to open up, even to themselves, for fear of being seen as weak or unattractive. From boyhood we are told to be brave and ambitious, overcoming obstacles on sports fields, battlefields, boardrooms. But we don’t see many of the mental challenges we face in this way. They are seen as weaknesses that shouldn’t be there, rather than parts of everyone’s lives.

We will all face grief, loss, and relationship issues, and these challenges take their toll, so we’d do well to seek to equip ourselves with understanding.

There’s a Leonard Cohen line I often think of, when he went on stage and couldn’t perform for some reason. He said he didn’t feel strong enough because “somehow the male and female parts of me refuse to encounter one another tonight”. I like the idea of acknowledging that both need to combine in an individual for a person to function. The same is true of society. By balancing notions of strength which tend to be thought of in a binary way, as either feminine or masculine, people might be much healthier. A world without sexism would be better for everyone. Not only would women be respected, but men might be better able to appreciate more feminine traits and strengths, in others and in themselves.

Why We Need to Talk About Men’s Mental Health

As Men’s Health Month comes to a close, I want to talk about something that we don’t talk about enough: men and depression. While it’s not exactly a women’s health topic, our typical focus at the Office on Women’s Health, we know that most women have important men in their lives. Fathers, partners, sons, brothers, uncles, and friends. Their health matters to us. They’re why we need to have a conversation about mental health — because I worry they’re not talking about it enough.

Depression can’t be willed away. It’s a serious mental health condition that affects a man’s daily life, including the way he eats, sleeps, feels, and thinks. It can also affect his ability to work, go to school, and maintain relationships with friends and family. Depression is NOT a sign of weakness, and it can affect any man, regardless of his age and race or ethnicity.

What would cause a man in your life to develop depression? According to the National Institute of Mental Health , it can be a combination of factors. For starters, his genes can play a role. Men who have a family history of depression are more likely to develop it than those who don’t. Environmental stress can trigger depression too. This includes financial problems, the loss of a loved one, work problems, a difficult relationship, a major life change, or a stressful situation. Also, depression can occur in men who have a serious medical condition like diabetes, heart disease, or cancer.

Getting an accurate diagnosis and following it up with treatment is essential. Most people need professional help to feel better. Without it, depression can lead to serious consequences, such as trying to self-medicate with drugs or alcohol, or worse — suicide. Suicide is one of the leading causes of death in the U.S., and men die by suicide at rates almost four times higher than women. Unfortunately, a recent report by the Centers for Disease Control shows that suicide rates are on the rise across the nation.

The report looked at data from 2015 to compare deaths by suicide among people with and without known mental health conditions. In both categories, men were more likely to die by suicide (84% and 69%, respectively). While depression and other mental health conditions aren’t the only risk factors for suicide, many of the issues that trigger depression can also contribute to suicide. Those include problems with health, money, and work. Substance use and problems in relationships can also contribute to suicide.

My message this Men’s Health Month is simple: We need to talk openly about mental health. Suffering in silence isn’t a safe or healthy option for the men in our lives. Check in with them. Ask them how they’re feeling, especially if one of them has experienced a recent event that could trigger depression. Look for changes in his behavior and watch out for depression symptoms. If you think someone you love has depression, offer your support, listen, and be patient. Encourage him to talk to his doctor or a mental health professional, or you can help him find treatment services in his area . Living with depression can feel overwhelming and isolating, so let him know he’s not alone. Help is available, and he can feel better.

Why Talking About Mental Health With Men Saves Lives

For most people, opening up about mental health isn’t fun. It can feel vulnerable, uncomfortable and — let’s be real — terrifying, especially if you have any dark thoughts. 

And while many people struggle to talk about their mental health, data shows men aren’t talking enough about it.  

“Men, especially middle-aged and older men, are the groups with the highest rates of suicides,” says Dr. John K. Amory , a physician at the General Internal Medicine Center at UW Medical Center – Roosevelt .  

In fact, people aged 65 and older  are more likely to die by suicide than any other age group — for both men and women. 

So, what needs to change? Well, both men and their loved ones need to try to break the silence and begin talking about mental health — before it becomes don’t-do-or-die. 

Men’s (mental) health crisis 

Let’s be honest: Fewer men generally access their healthcare options than other genders, not because there aren’t resources, but because many men opt out of their routine healthcare. 

“Some of my male patients are very good health advocates for themselves. Others are certainly … less so. In those cases, I do think there’s a mental construct of ‘self-sufficiency’ for some men that prevents them from asking for help,” says Amory. 

In 2021, the overall life expectancy for men dropped from 74.2 years to 73.2 years. And to bring this back to mental health, the rate of suicide was the third-most reason for this decline in life expectancy, only behind deaths from COVID-19 and unintentional injuries. 

Out of these causes of death, only one is entirely preventable, and that’s suicide.  

Knowing the facts can help give some perspective on why this out-of-sight, out-of-mind perspective isn’t helping men’s mental health.  

How mental health conditions affect men  

According to Mental Health America , these top mental health problems affect men: 

  • Depression: over 6 million men suffer from depression. 
  • Anxiety: 14.3% of men have a form of anxiety disorder .
  • Bipolar disorder: 2.9% of men have bipolar disorder . 
  • Psychosis and schizophrenia: 90% of those diagnosed with schizophrenia by age 30 are men. 
  • Eating disorders: 10% of patients with anorexia or bulimia are men, and 35% of patients with binge-eating disorder are men. 

While these are the general statistics, the truth is, many men who suffer from mental health conditions or eating disorders remain undiagnosed, leading to their conditions worsening. It’s thought that the perceptions around gender and the way men are socialized from a young age are part of the issue. 

This could mean that men mask their feelings of depression or anxiety because of the stigma around men and their emotions. Needing help isn’t seen as “masculine,” but suffering in silence shouldn’t be the answer. Men of color are even less likely to seek or receive help .  

“There’s an ethos that men should ‘tough it out’ and that getting mental health care isn’t ‘manly.’ Obviously, this approach is not working,” says Amory. 

It’s also untrue that men don’t want to talk about their mental health or other struggles. Because of these gender socializations and, sometimes, personal preferences, it’s just about creating the right environment to talk to men about their mental health in a way that feels safe and organic. 

How to start the conversation  

The research shows that many men cope with distressing emotions differently  than other genders and are less likely to seek help .  

If your loved one isn’t talking about his mental health issues, it can be hard to find a place to start. And while you might be coming from a place of worry, support and love, remember that this conversation isn’t about you or how you’re feeling. Steer clear of accusations around their actions or feelings, and instead, start with observations.  

Tell them what you’ve been noticing. This can look like: 

  • I’ve noticed [behavior or feeling] lately and wanted to check in. 
  • When did you start feeling this way? 
  • How are you managing? 
  • Do you have anyone to talk to about this? 

Validate how they’re feeling, but don’t offer advice. Instead, you can offer support and resources. 

Suggest a trip to the doctor 

Going to the doctor might not seem like mental health care, but it’s possible that a medical condition, such as low testosterone, could cause some symptoms of depression. Anxiety and other symptoms could also be side effects of medications or medical conditions, so a full workup is a great place to start. 

If it’s his first appointment to talk about his mental health, offer to go with him. If he’d rather go alone, let him know you’re there for him when he needs support.  

When patients come into Amory’s office with concerns, he says he tries to do a lot of the heavy work in facilitating the conversation to make his patients feel more comfortable.  

“I often see men visibly relax after we start to discuss these issues openly,” says Amory. 

But if the doctor doesn’t seem to be leading the conversation, encourage your loved one say something clear and honest  like, “I’ve been having a lot of anxiety lately, and would like to know what my options are.” 

Ask what he needs 

Everyone opens up on their own terms and requires different methods of support. Don’t assume someone needs a specific type of help, such as medication or therapy. In addition, don’t be afraid to directly ask: Are you thinking about suicide? It’s a myth that the question will give them the idea. In fact, it could give them the much-needed opportunity to talk about their thoughts. 

If he won’t open up 

You can say and do all the right things, but at the end of the day, it’s up to your loved one to decide when they’re ready to open up and seek support for their mental health. Someone might not open up  for a variety of reasons, from not knowing what to say, to shame or fear.  

And while all of this might be frustrating, remember — this is about him, not how you’re feeling. Respect his decision to not open up, and in the meantime, you can continue to be a support by offering a safe space to talk when he’s ready, by spending quality time with him, or offering to help him with tasks.

Finally, if someone tells you they’re thinking about suicide , have an open and honest conversation. From there, you can work with them and their loved ones to decide the best course of action.

Talking about mental health is an extremely vulnerable act, but it can make a huge difference in someone’s life — it might even help save it.

Changing how we talk about men’s mental health

The rate of mental health disorders between men and women is about the same, yet fewer men access professional help or a service. Men are also more likely to die from destructive coping mechanisms like suicide, drugs and alcohol. Why is that? And can changing the way we talk about men’s mental health make a difference?

  • Providing support

Did you know that more than 45% of Australians aged 16-85 years have at some point in life experienced a mental illness? Yet 65% percent of people who are struggling mentally don’t take serious steps, like seeing someone about it[1]?

And did you know that men are less likely than women to actively take steps to access a service or some kind of professional help, even though the rate of mental health disorders is about the same? We know this because men account for only 40% of Medicare-subsidised mental health services in Australia[2]. That means there are a huge number of men out there who aren’t getting help.

We also know that a man is more likely to die from destructive coping mechanisms like suicide, drugs and alcohol.

For example, did you know:

  • The number of suicide deaths is approximately three times higher in males than females[3]
  • Men are more likely to die because of alcohol and drug use than women[4].

These aren’t insignificant figures. Why is there such a shocking difference?

As with so many things in life, the reasons are many and complex. A range of psychological, physical and environmental factors can contribute to the problem. Social background, economic factors (such as unemployment), physical health, a family history of mental illness, alcohol and drug consumption, childhood and family stability and even some genes are among many of the things known to play a part.

Even so, the way in which many men deal with their stresses and worries is thought to be one of the underlying causes. A lot of men are reluctant to talk to or see someone about their concerns. They may feel it is somehow a sign of weakness, that their worries aren’t serious enough, or that they’ll be judged by their friends and family.

Thankfully, there’s good news. There is more research and evidence than ever that shows support programs can make a huge difference to men if they are based around their specific needs, preferences and strengths.

How can we support change?

New ways to provide support.

In the world of professional mental health care there has been a big increase in the number of ‘non-traditional’ ways that counselling is provided. In recent years we’ve seen not only phone counselling services but also online and other digital services become more popular.

In a large country like Australia, these are great ways to make up for huge travel distances, especially in rural or remote areas where there might be limited services.

However, closing travel distance isn’t the only reason why these services are more widespread. What is also getting more recognition is that these forms of support can work well for people who might like to try something other than being ‘on the couch’.

If more men who could do with support (but who wouldn’t opt for the face-to-face option) take up these new services, then that means more people get the help they need.

Change the message

On MensLine Australia, counsellors are trained to use a style that’s based around communicating about goals and solutions, rather than just ‘talking about feelings’.

An example is in the way language gets used. Counsellors might refer to something as “mental fitness” instead of “suffering from mental health issues”.

Talking about obtaining help and support as a show of strength, taking control and getting things back on track can in turn change the way someone feels about mental health. That means asking someone if they’ve been “struggling with” or “battling against” pressures rather than “feeling sad or depressed”. Even thinking of mental health as “stress” increases the likelihood of men accessing support and taking on those suggestions that can improve their situation.

Support specifically for men

Another way that services are changing is by changing the approach to providing support.

For example, communication style is a big one. You probably know that men and women tend to communicate differently. Incorporating sometimes surprisingly simple concepts such as a change in stance has been shown to make communication easier and more effective.

One way of doing this is the ‘shoulder to shoulder’ style of communicating, which tends to suit men better. Many men opt for this style of talking when it comes to serious discussions. A characteristic of this style is that eye contact is usually minimised — think of guys discussing things while watching sport, gaming or fishing. Recognising how small but significant things can improve communication is in itself an effective way to improve the success rate of support services.

Changing the environment

Another concept that’s been shown to be effective is the concept of activities in specific places. The idea is that a conversation that touches on things like emotions and mental health will be more effective if it takes place in a ‘friendly’ or familiar environment, such as in a sporting or outdoors location.

There are many reasons for this but they all centre around the principle that a ‘better’ environment means men are more likely to open up and be receptive to support.

Beyond traditional help

There is a growing body of evidence that shows that new approaches can greatly benefit the way in which we manage men’s mental health.

The mental health profession needs to look beyond the traditional ways in which support is delivered. New and innovative ways of providing help will make it easier for men to get the help and support they need. The health of our society depends on it.

Men's Mental Health Stigma: A Male Issue or a Social Issue?

Men’s mental health stigma remains overlooked and ignored, and still lurks silently in the shadows of society. Importantly, stigma is typically divided into two dimensions: internal stigma and external stigma. Internal stigma refers to private feelings of shame and worthlessness held by stigmatized individuals, often leading to social withdrawal and low self-esteem . External stigma refers to negative attitudes and prejudicial stereotypes held by people at large, often leading to rejection and subsequent discrimination .

Importantly, the discourse surrounding men’s mental health tends to focus on the internal stigma of affected men, who are often depicted as self-destructively stubborn and silent in the face of mental illness. This narrow focus can sometimes descend into a victim-blaming discourse. As such, it is important to broaden the focus to consider external stigma by examining negative attitudes and prejudicial stereotypes elsewhere in society.

This external stigma may be a key factor deterring men from discussing their mental health issues and seeking help from services, and is found in surprising places.

Men’s Mental Health Stigma in Health Services

The academic literature indicates that men are significantly less likely to use mental health services than women in the face of a mental illness. The reasons for such under-utilization are varied, but external stigma is a huge and under-acknowledged factor. Indeed, some research indicates that health care providers themselves can hold stigmatizing attitudes towards people with mental illness.

For example, one survey found that 44 percent of service users report major discrimination in their interactions with the mental health care system. In a study of low-income men living in rural New England, participants reported numerous negative encounters with clinicians, with one stating that "Whenever I would go in there [a local hospital], they treated me like a drunk, and I don’t drink!"

More recently, the American Psychological Association released "guidelines for psychological practice with boys and men" which states that "conforming to traditional masculinity ideology has been shown to… negatively influence mental health." The guidelines were severely criticized by a range of experts, who argued that the document stigmatized and pathologized masculinity, thereby missing an opportunity to reform and innovate approaches to men’s mental health.

Men’s Mental Health Stigma Within the Family

Research indicates that the immediate family can be an important source of support and solace for people with mental illness. That said, evidence suggests that some family members can perceive mental illness as a source of shame that is ruinous to familial reputation. This can lead to attempts to deny or conceal the mental illness, again deterring help-seeking.

Parents, children, and even spouses can be complicit in this silence, which may be particularly intense if the family member was a man. For example, my colleagues and I conducted a research study finding that husbands with mental illness frequently reported negative and unsympathetic comments from their wives. This was especially so if the mental illness interfered with the man’s bread-winning abilities, leading some wives to question his masculinity. "I lost the respect of my wife as I continue to fail in my duties. She lost trust in me. I am extremely sad when I hear the sentence 'Are you a man?' and 'Are you a husband?' from my wife. I want to die. I want to commit suicide ."

Men’s Mental Health Stigma in the Media

It is well-known that media portrayals of socio-economic groups can shape wider public attitudes and beliefs. In a recent study , my colleagues and I examined media coverage of mental illness by gender , finding that articles about women with mental illness tended to be more positive and empathic , while articles about men with mental illness contained more stigmatizing content, and linked men’s mental illness to crime and violence.

This is an important finding, as these media portrayals can contribute to a wider climate of fear regarding men with mental illness, which can deter help-seeking. For example, men may legitimately fear being stereotyped by their family, friends, and colleagues as prone to crime and violence if they disclose mental health issues. Indeed, this is a common sentiment among men with mental illness, summarized by a research participant below:

"People, they have fear... They think, 'That man there, he is crazy. He could kill us!' They think, 'He is not like us!' They think you are lower than them. If I say to some, 'Yes, I am schizophrenic,' for sure they are going to have another idea of me. I am scared of saying that to people. I do not want them to judge me."

Discussions about men’s mental health typically adopt a narrow lens, focusing on the supposed silence and stubbornness of men with mental health issues. However, evidence suggests that it is vitally important to widen this lens to understand the broader social context in which men’s mental health issues occur.

Men may be silent in the face of mental illness due to external stigmas and social stereotypes purveyed by disparate groups including health care professionals, family members, the media and other elements of society. If we are serious about tackling men’s mental health issues, then this external stigma must be documented and addressed.

A narrow approach will solve little.

The first thing that you should know about mental health is that there are many different types of disorders that fall under this category. The most common are depression, anxiety, bipolar disorder, schizophrenia, borderline personality disorder and obsessive-compulsive disorder (OCD). You may have heard of some of these before or seen them in the media but there are many more types than just those listed here.

The second thing to know about mental illnesses is what causes them. There are different factors that can cause someone’s brain chemistry to change which could leave a deep impression on one’s life.

Anyone, regardless of age, gender, or background, can have mild to severe mental health issues. A good mental state is crucial for many reasons. It aids us in overcoming difficulties and tensions that we face on a daily basis.

The topic of mental health has to be discussed.. It is an issue that affects people from all walks of life, and it is an issue that needs to be addressed. The stigma surrounding mental health has been present for a long time, and it is still present today. People are afraid to talk about mental illness because they are afraid of what other people will think of them or how they will react.

In the past, people who were dealing with mental health issues were often stigmatized and made to feel ashamed. Mental health is not a disease that's easily understood by the general public. It's something that we're learning more about every day, and it's something we're all responsible for learning about. Mental illness is not a sign of weakness; it is a sign of strength. It takes courage to face your mind and to work on improving your mental health.

Trauma is also a cause for bad mental health and this could be something that happened in the past or something happening currently in your life such as abuse, bullying, or neglect. Here are some points on how to take care of your mental health:

Develop a routine and stick to it. This will help you get into a healthy routine and make your life more manageable.

Practice self-care by doing things that you enjoy such as reading, drawing, writing or going for walks. Self-care can also include taking time off work, getting enough sleep and eating well.

Take care of your mental health by being mindful of what you're feeling - don't ignore it or try to avoid it because this will only worsen it.

You're not the only person who has these feelings and this is not your fault.

Even if you don't feel like going out, it's important that you do so to maintain a sense of normality for yourself.

Opinion: Why we should all care about Black men’s mental health

Like many Black men, I grew up with a stern father who saw displays of emotion as a sign of weakness. For a long time after becoming a father myself, if overwhelmed by unhappiness or frustration, I would hide my own tears from my little boy.

Eventually, I realized I had to break the cycle of repression. Becoming a parent and thinking through how I was parented led me to handle my emotions differently, so that my son wouldn’t feel ashamed to show his.

Now my young son knows that men are allowed to cry and that difficult feelings can be expressed and overcome. I hope this will help protect his mental health when he’s older, but as a Black American male, unless things change dramatically, I fear he will encounter a unique set of challenges.

The theme of this year’s  World Mental Health Day  observed on October 10 is “Mental health is a universal human right.” I want myself and my fellow Black American men to claim this right, for the sake of ourselves, our families, our communities and our society.

In the United States, 1 in 5 adults experience mental health difficulties each year, and 9 out of 10 adults believe our nation is now in the throes of a mental health crisis,  according to a survey  taken last year.  Millions of American  men suffer from depression, anxiety and other mental health disorders, and are far less likely than women to seek help.

Men are nearly  four times as likely  as women to die by suicide. White men are the group most likely to take their own lives, but from 2014 to 2019, Black people had the largest percentage increase of any ethnic group in  suicide death rates – a rise of 30 % – putting Black males at particular risk given the greater proclivity towards suicide among men.

Black children between the ages of 5 to 12 are also at higher risk: They are twice as likely to die by suicide as their White peers. And there has been a  60% increase  over the past two decades in suicide rates among Black boys between the ages of 10 and 19, which I believe constitutes a national emergency.

As shocking as all these figures are, I don’t need statistics to tell me there’s something very worrying going on with Black men and their mental health. I can see it all around, including in myself.

Having wrestled with depression my whole life, I recently found myself at a new low after experiencing a series of bereavements. Among those departed loved ones are two Black men who tragically took their own lives. By trying to stay strong for others instead of acknowledging my own need to process my grief, I made myself sick. In desperation, I sought — and was eventually able to get — professional support.

I am privileged to know a successful, high-functioning Black man who lives with Borderline Personality Disorder (BPD), a complex mental health condition. Men like him are often labeled the ‘angry Black man’ rather than being properly diagnosed and treated.

A mix of negative factors expose Black men to mental health difficulties while making us particularly reluctant to seek treatment when we become unwell. This is compounded by the fact that even when we do accept we need help, we face disproportionate barriers to accessing good quality, appropriate mental health care.

Often our struggles are rooted in  racial trauma  that can be traced back to both historical experiences, such as slavery and ongoing racial discrimination. My father was not alone in acting tough — hiding his emotions from me and my siblings, removing himself completely whenever he felt angry, sad or upset — as a coping mechanism. Oppression and marginalization leads many Black people to grow an invisible shell in the mistaken belief that if you don’t allow yourself to have feelings — or at least don’t appear to be vulnerable — daily brushes with the myriad brutalities of individual and systemic racism can’t hurt you.

Highly traditional gender roles persist in the Black community, stigmatizing men who express vulnerability as shamefully fragile. And if you’re born into a culture that promotes hypermasculinity, you are in danger of developing toxic attitudes and behaviors that can exacerbate issues like depression, anxiety, borderline personality disorder and substance abuse, leaving you even more isolated. Our collective coping mechanism, it turns out, is just adding to the harms inflicted on us.

Yet, for many Black men, recognizing that they need mental health support is only the first step on a long journey to wellness. The first hurdle they must overcome is an internal one — a deep-rooted  distrust of health services,  stemming from historical medical mistreatment and modern-day discrimination in health care.

External hurdles also come at Black men hard and fast. And when treatment is provided, it is rarely by practitioners equipped to meet Black men’s specific needs. The shortage of Black mental health specialists in the US — only  5% of psychologists  are Black — makes it extremely hard for us to access truly culturally competent care.

In an advanced economy like the US, it is simply unforgivable that racial injustice robs a significant demographic group of their right to good mental health. We must fix this, and we can.

Black men should work together to increase awareness within our community, taking vital messages about mental health to key spaces where men gather, such as places of worship, sporting events — even at the barber shop. I draw inspiration from the  Confess Project of America , a national initiative that fosters emotional wellness in the Black community, and that has trained more than 3,000 Black barbers and beauticians to act as mental health advocates.

Government can play a role too. The US needs national public health campaigns targeting Black men, the expansion of Medicaid to reduce disparities in access to care, and more mental health clinicians trained in gender- and race-sensitive treatment, building on the good work of organizations such as the  Black Mental Health Alliance. 

We have an ethical obligation to commit to achieving racial equality, but we should also remember the countless practical advantages of doing so. Better mental health for Black men is just one of these, but it is one that will undoubtedly benefit the whole country, for when men’s distress is ignored, their ability to contribute fully as citizens is severely curtailed.

If you’re the father of a Black son, don’t be afraid to cry in front of him. Let that be our gift to America — empowering a new generation of Black men to have the courage to share their feelings and the strength to ask for help.

And if you are struggling or have ever struggled, find another man — Black or White, trans, gay or straight, impoverished or wealthy —  and please tell him about your mental health challenges. Don’t put on a brave face. Be really honest. Then ask him how he’s doing and —  whatever his reply — give him a hug.

At the center of the criminal case against former President Donald Trump in Manhattan is the accusation that Trump took part in a scheme to turn The National Enquirer and its sister publications into an arm of his 2016 presidential campaign. The documents detailed three “hush money” payments made to a series of individuals to guarantee their silence about potentially damaging stories in the months before the election. Because this was done with the goal of helping his election chances, the case implied, these payments amounted to a form of illegal, undisclosed campaign spending. And because Trump created paperwork to make the payments seem like regular legal expenses, that amounted to a criminal effort at a coverup, argued Alvin Bragg, the district attorney of Manhattan. Trump has denied the charges against him.

For Lachlan Cartwright, reading the indictment was like stepping through the looking glass, because it described a three-year period in his own professional life, one that he has come to deeply regret. Now, as a former president faces a criminal trial for the first time in American history, Cartwright is forced to grapple with what really happened at The Enquirer in those years — and whether and how he can ever set things right.
