Cardiovascular diseases are worldwide a major challenge for the health care system and the number one reason for premature mortality and lost life years. Many accomplishments to reduce risk factors and improve treatment in acute and chronic disease and rehabilitation have been initiated by medical research but were also embraced by nursing research, which provided valuable knowledge about supporting risk factor modification and patients' self-management. Nursing research in the cardiovascular field has a long tradition in the US. In Europe cardiovascular nursing research also developed over the past decade, with increasing participation of Swiss nurse researchers. Efforts in this field of cardiovascular nursing have been made to convey projects in nursing research that help to enhance the health of the population sustainably and improve well-being in patients with acute and chronic courses of cardiovascular disease. Scientific knowledge is pivotal in developing evidence-based nursing interventions. Since both the German and the French speaking part of Switzerland have been lacking a literature-based and expert-supported agenda for nursing research, the aim of the Swiss Research Agenda for Nursing (SRAN) project was to develop an agenda providing researchers, funding agencies, and politics with orientation. This article takes the seven priorities of the SRAN project and links them to the topics of risk factor modification, rehabilitation programs, self-care, and patient education. The article presents the first agenda for cardiovascular nursing for the years 2007 to 2017. The agenda will serve to develop an action plan and to promote nursing research projects in the field of cardiovascular nursing in Switzerland.

Cardiovascular disease (CVD) remains the leading cause of death and disability in most countries of the world, including Russia. Mortality from CVD is particularly high in the Russian Federation compared with the average in Europe (55.7% vs. 46%). 1,2 In Russia 29.4% of deaths occur from coronary heart disease (CHD) and 17.6% from cerebrovascular disease, mostly strokes. CVD kills more women than men: 51 and 42% respectively in Europe, 60.2 and 47.2% in Russia, with women dying at older ages.

A decline of CVD mortality started in Russia in 2003 and has become more stable and pronounced since 2006. In 2005 a new state program was launched called National Priority Project ‘Health’, aimed to both increase the availability of modern treatments and interventions for patients with CVD and to promote active CVD prevention in primary care. Since 2009, 695 Health Centers with a special focus on primary prevention started to work in all regions of the country (65 in Moscow).

Between 2005 and 2013, age-standardized CVD mortality rates in Russia decreased by 34.3% (833.2 vs. 547.1 deaths per 100 000 population) and CHD mortality rates—by 24.7% (383.6 vs. 289.0 per 100 000 population) with significant differences between country regions. 3 The most favourable changes were seen in Moscow. Whereas standardised CHD mortality rate decreased by 35.7% in Moscow (306.1 vs. 196.9), the decrease was 1.3 times less in St. Petersburg (362.1 vs. 258.9), 2.6 times less in the Moscow region (433.8 vs. 374.3) and 1.5 times less in the Russian Federation.

The mortality rate improvements were accompanied by a rise of CVD morbidity: the number of patients newly diagnosed with CVD in the Russian Federation increased by 15.9% from 2005 to 2012. This was most probably caused by improved screening. In 2013, owing to a large-scale screening programme (called ‘Dispanserization’), the number of patients newly diagnosed with CVD increased in Russia by 13.7% in just 1 year (as compared with 2012). 3

In the frame of this program, 2 003 099 Moscow inhabitants were examined in 2013. Of those, 34.7% had low or intermediate risk by SCORE scale, high (≥5 SCORE) and 36.6%—cardiovascular and other non-communicable diseases. The program also included risk factor counselling and management.

In 2013, the life expectancy of Moscow inhabitants reached 76.4 years (Russia's average—70.8 years), with only 4 years separating them from residents of the EU. Significantly lower CVD mortality and higher life expectancy in Moscow city in comparison with the average national figures are likely due to the higher socioeconomic standard of living, better psychological stability and availability of mental health services, higher consumption of fish, fruits, and berries, and better availability of outpatient and high-tech medical care for residents of the capital.

To achieve further sustained reduction in CVD mortality and increase in life expectancy, the implementation of a comprehensive complex of preventive measures aimed at promoting healthy lifestyles, reducing risk factors, implementing primary prevention of CVD, and optimising healthcare to address secondary prevention of CVD is required.

The Bulletin was founded in 2000. The objectives of The Bulletin of Bakoulev Center include coverage of diagnostics issues, conservative, interventional and surgical treatment of patients with congenital and acquired heart defects, ischemic heart disease, vascular disorders, terminal heart failure, interactive pathology, as well as their rehabilitation.

Among the Bulletin sections are reviews, original articles, brief reports, lectures, history of science, scientific achievements and other.

The articles by experts of the National Medical Research Center for Cardiovascular Surgery containing in-depth analysis of the Centre activities as the leading institution of the industry in various scientific and clinical areas as well as analytical reports of its separate structural units are published here. The journal provides authors from all regions of Russia and from abroad pages for publication of their materials, thus becoming a platform for scientific discussions, exchange of information, sharing of experience.

The Bulletin is included in the list of peer-reviewed scientific journals recommended by the Higher Attestation Commission for publication of basic results of dissertations for candidate and doctor of medical sciences degrees in the specialties: 3.1.15. "Cardiovascular Surgery", 3.1.20. "Cardiology", 3.1.21. "Pediatrics".

Study provides guidelines to doctors for individual treatment of wrist fractures

A doctoral thesis by Viktor Schmidt, Umeå University can hopefully help doctors to provide better information to patients after wrist fractures. It can determine which treatment is most appropriate for each individual patient.

The thesis shows that the alignment in which a wrist fracture heals in is important in terms of early results (1 year after the injury). After that, there is a continued gradual recovery, and a decade after the injury, most patients are fully recovered in function, regardless of the alignment in which the fracture healed. By identifying unstable fractures and following guidelines that promote early surgery, short-term outcomes are likely to improve.

A broken wrist, or distal radius fracture as it is called in technical terms, is the most common skeletal injury. A well-functioning wrist is important to be able to work and engage in activities of daily living. Treatment after such an injury is aimed at restoring function by recreating anatomical alignment and providing the patient with adequate rehabilitation.

The first study examined results one year after injury.

"We could see a correlation where malalignments led to poorer results in terms of range of motion, grip strength and self-rated problems," says Schmidt, Department of Diagnostics and Intervention.

The second work analyzed whether a continued movement in the skeletal injury between emergency room visits and return visits had any impact on stability.

"We found that this movement, which we have chosen to call 'marginal secondary displacement,' is an important parameter to take into account in order for the skeletal injury to heal in a good position."

The third study followed up on the patients from the first study, a decade later. It showed that the results continue to improve over time and that after more than a decade, the patients were restored in terms of range of motion, grip strength and self-reported outcomes, regardless of the position in which the bone injury had healed. The final study investigated the potential consequences of the fast tracks introduced in the Swedish national guidelines of distal radius fractures.

"We compared the treatment that was given with how they would have been treated if the guidelines had been followed. We found that if the fast-track program had been followed, more patients would have been treated with early surgery and a delayed operation could have been avoided in the majority of cases," says Schmidt, who defends his thesis on May 8.
